IC - AFRCC initial contact checklist

Any of your wings have a formal checklist for the IC to use during the initial conversations with the AFRCC (or other agencies?

We're looking to develop one but would prefer not to build from scratch if some wings have a good one.

Eclipse  --  As I was working toward my IC qual, I drafted a multi-page worksheet for the initial call out conversation with AFRCC.  It has been disseminated within the wing and is in use by a number of our IC team.  It was first used during our SAREVAL earlier this year, and received favorable comments from the evaluation team.  This specific worksheet is directed at missing aircraft missions.  When I can find the time, I intend to prepare something similar for other potential missions types, to include missing persons, ELT, etc.

IN GAWG AFRCC intital contact is with an Alert Officer. Each Alert officer takes a two week duty period where they are on call if something happens. AFRCC calls the alert officer and they call the nearest IC depending on where in the state the incident is, then the IC calls AFRCC to make coordination.
