CAP Form 500???

Has anyone heard of a form 500 or know where to find one?  At a meeting tonight, two senior members said that if a cadet is attending a function with a squadron other than their own, they should fill in a form 500.  I have lots of CAP experience, but I must have missed this somewhere.  As usual, thanks in advance.

CAP Forms only seem to go up to CAPF 176, unless it is a supplemental Wing form of some sort. ???

I've never heard of it, and it's not on e-services.

There is a FLWG Form 500, which is the general equivalent of CAPF 31, but for use inside FLWG. It sounds like they are referring to that, or a local equivalent.

Could they mean a CAPF 50?  That would be a Cadet Progression Evaluations.  It's required to be completed at least once per phase.  I've also heard of it being required for NCSA applications & staff applications for some encampments/local special activities.

I love it when a Wing makes up a form when there is already one available that does the same job.

I wonder if they meant a CAP F 60? Emergency contact info.  I know in my experiences, most seniors don't know what it is.  We got gigged on a CI a few years ago because most members at that time didn't have one on file. 

Anytime you're away from your unit, you're supposed to have one in hand. Lately, all activities in this wing, it's a requirement.
