CAPF 45b is missing

The CAPF 45b is missing from E-services, and I lost my copu of it.  If anybody could post it or email it o me, I would be grateful

Well, this is new; the CAPF 45 now has the old 45b merged into it.  The things I learn after being out of the loop for 6 months. . . .

I could still use the old 45b though. . . .
