Electronic Form 45

Has anyone figured out a convenient way to do electronic Form 45s (and F66, for that matter) for personnel records?  Say in Google Docs or whatever? 

I uploaded the Word version to see what would happen and it doesn't work very cleanly as a converted document. 

I think I've got the Specialty Tracks figured out, to make those worksheets electronic.  The forms 45/66 is more of a challenge.

We had a few old forms 66 and 45 and just went to Office Max and had them copied on legal size card stock (double sided).

I have suggested to national that they should create a auto fill forms where we can just enter a CAPID and all the WMIRS and CAPwatch data auto fills form. This layout should be the used in e-services.

I created a Form 45B from scratch in FileMaker Pro, as a searchable database that also populated the form. A couple of months later the form was discontinued. I started working on a 45 but gave up.

The reality is that the 45 can't accommodate enough data.  Nearly everything is tracked in eServices.  I generate reports from eServices so that data is kept somewhere else. 

I really don't particularly like the 45 or particularly want them, they're just an inspection item.  Our unit database keeps better records.

Our squadron was told after an SUI inspection you only need to record information on the CAPF 45s and 66s if it is not in eServices so, at this point we are printing Members Master Records and printing these 45s and 66s off the CAP website and there isn't much members have to enter. We then retain both in their Personnel files.

That is the personal opinion of the individual on the inspection team. Not that I disagree, but the next inspector might have a different opinion.

Agreed, however at this point, after taking over a very neglected squadron and needing to prepare for an SUI, I will go with this advice and work on repairing things over time to include complete records.

Essentially, the only things on a Form 45 that aren't in eServices are:

       
  • SOME awards and decorations
  • Education (civilian/military schools/degrees)
  • Flight Officer promotions
  • Conferences
  • Presentation to a non-CAP group
  • Course staffing/director credit
  • Activity Participation (we don't use this page at all.  It's not big enough and that's what PLs are for)
Most of these items are handled through certificates or Participation Letters.  We don't need to track what high school or college/university they attended. 

The awards section isn't productive.  We have certificates and the My Ribbons feature in our online personnel records system.

The problem is "your" online personnel records system is not the national database, and is not accessible by others.  So the awards system may not be productive for you, doesn't mean its not useful for others.  Especially those, who, like myself, move around a lot

For the purposes of PD awards, these actually are tracked in eServices. Unfortunately, it requires someone at the squadron to go in and enter them and they aren't visible on the member record. But they are technically tracked.

Not too sure what that means.  The Form 45 is not the national database, and is not accessible to anyone who can't get into the filing cabinet.  Except in our case, in which the Form 45 is online, each member has view-only access to their complete record, including the Form 45 at any time, from anywhere they like.  All the unit's Form 120 awards are in eServices, all 2a awards are on the Form 45 (which only serves as a summary - not approval), authorized on a 2a in their Personnel Actions folder, and viewable at a glance by clicking on My Ribbons.
