Generating a 45

I want .. (the database, that is.. ) I'd like to set that up too, to have a similar tracking report like the cadets.

Is there any way to edit the fields on a CAPF45.  Not the Data entry fields already provided.   But someway to change printing fields and edit the currently locked fields.  The Form is out of date and really doesn't suffice for my needs.  The requirements require I use it, but I want to edit it so that I can create a worksheet to verify and correct the information.

I'm still using the CAPF45 as intended in its original form.  I want to create a Worksheet that I can print with printing four sheets of paperwork.  My plan is to edit and either remove fields or reduce the number of data entry fields so I can use it as a checklist so that it makes verification of records easier for me.  I doubt the NHQ Forms Gestapo will be out to get me.
