Per mission slip Need examples

I am looking for examples of language for cadet activity permission slips.  If anyone could share some, I would appreciate it.  Thanks.

Just describe the event, location, approx time returning, materials and expectations.

I suspect the question being asked is related to hold harmless and disclaimer type text, on a "per mission" basis ;)

Yes.  We've never used a permission slip.  I want to start, as it is required.  This is for activities such as squadron campout, Tour of AFB, etc.  There seems to be two schools of thought--just a simple permission to go, or a release of liability.  Any thoughts on this subject appreciated.

I'd have to dig it up, but we had a squadron form that was basically the CAPF 60 with some additional info on the back, to include the ENTIRE text of the waiver from CAPF 31 and places for mom/dad to sign. 

Anything outside of our normal AO, anything out of state, or overnight and we had this form accomplished.

Thanks, Col.  I'd thought of doing the same thing with CAPF 60 language.  I think I'll just draft one that way and run it by the wing legal officer.  I think it would be a good idea if CAP made a form for this purpose.

Permission slips, many times, are not legally binding unless they are notarized and will not prevent litigation in many cases. 

I tend to use them as informational means to let parents know what is going on.  The signature shows that they know where their child is, who they will be with, what they are going to do, where they might go and any financial support needs (meal money, equipment et al)

It also prevent that occasional happenstance where a parent had no idea CAP was going on a field trip and suspects that their child is missing.  :P

Concur, to a degree....see my previous post on what we use such a thing for.

Very few activities require a permission slip - most permission for activities is implicit with membership, however for those that do, the Form 31 is plenty.

Unless you run the form by your wing's legal officer, they may not have any value, or might actually get the organization in trouble. 

Don't use one from another wing without local review.
