High risk activity

New safety guy here.  I have to do a 160 form for a high risk activity.  Is there a guide available to help fill this form out?
Also does the Squadron Commander have to sign the 160 in addition to Wing?

There's not a guide to filling out the form, per se. However, and I'm assuming that this is for a cadet related HAA, the Girl Scouts have done most of the risk management work already.
