Title: MSA form needed

I'm putting together a tour of a local USAF base and I figured out that I need a Military Authorization Support form.

I read the regs and it seems that I'm supposed to fill it out? Even though it's on Air Force letterhead? Then pass it on to the region CAP-USAF office for signature?

Well, the format that's in the regs is difficult for me to duplicate in my open office application, so...

DOES ANYONE HAVE A PROPERLY FORMATTED COPY OF AN MSA THEY COULD FORWARD TO ME?

It really depends on your relationship with the State Director, but generally they create those forms.

My wing's SD usually wants a spreadsheet of names, CAPID's, SSN's, whatever the base requires, which he then attaches to his cover letter.

Your best bet is to call or email your SD to see what his SOP is.

+1  Call your State Director and/or your CAP-RAP and they will lead you in the right direction.
