Due to a rash of recent avoidable mishaps and reporting protocol errors, I am grounding the CAP fleet in NYWg from midnight, Monday, 11May2009, till midnight Thursday, 14May2009. The only flights allowed during this timeframe will be actual missions.
We are coming into our busy season of flying and cadet activities. I am grounding the fleet so that we can all take a serious accounting of the operational risks in the air, on the ground and at every activity from driving to a meeting, to exercising around fixed obstacles, or finding a damaged aircraft. We cannot take the safety of ourselves and others casually.
1. The grounding will be named Operation MINDSET RESET.
2. The GOAL of this activity is to wake up, maintain alertness and approach every tasking within this organization with precision from the minute you put on the uniform till the minute you hang it up as if a life depended on the correct completion of every task.
3. Self Assessment is key to maintaining alertness and safety.
4. Working within your limitations of energy and alertness are critical.
5. Understanding the limitations of the equipment and vehicles you operate is key.
6. Not accepting a tasking when you are not able to give it 100% is the correct judgment call.
7. Sharpening Situational Awareness is the objective.
8. During the next three days, aircrew will familiarize themselves with the NYWg, NER, and National Safety policies and notification protocols. All CAPF5 pilots will sign the attached form and send to the NYW DO before being allowed to fly after the timeframe is expired.
9. Squadrons will conduct briefings and exercises to address the above points.
10. Wing Staffers (CC,VC,COS,SE,DO) will refamiliarize themselves with their delegated roles in the NER CC Safety Policy.
11. NY DOV will contact all NYWg Checkpilots and Mission Checkpilots and instruct them to include, on all CAPF5 and CAPF91 checkrides, either a checkpilot tutorial on the protocol for reporting an Aircraft mishap, or the evaluated pilot explaining the information clearly and in detail. The information will include where to find the contact information and NYWg, NER and NHQ policies in the AC binders and explaining the sequence of events after a mishap.
The CAPF 5 or 91 will be annotated in the remarks box by the Checkpilot to specify that this explanation or instruction has been accomplished. "Safety procedures briefed". NYWg DOV will not revalidate any pilot that does not have this on their CAPF 5 or CAPF91.
12. This information will also be included in the NCPSC that will be held next weekend.
13. After this operation is completed, any pilot or staffer that does not follow the policies and protocols after a mishap will be grounded for a minimum of 30 days without exception.
This grounding is to underline and focus all our efforts on NYWg's Commitment to the SAFE conduct of all activities.

In my opinion this is the best point in the entire memo (not to cast any level of lesser importance on the others).
Many of us work long hours, have stressful jobs and might even have a beer after work. If you get a call at O dark thirty and you would not otherwise consider jumping into an airplane in any other circumstance, don't let your desire to fly a mission cloud your judgement!!!!
We all hate to say no when someone is trying to put a crew together but the best call you may ever make is to say "sorry, I'm not up to flying tonight."
Fatigue is a killer!!!
Well that is interesting. It is good to see a Wing CC taking proactive measures.
We're pretty lucky to have Col Andreu. He was an unknown quantity to me when he took command, but I've been nothing but happy with what he's done.

At least for pilots who have completed the Wing Commander's safety briefings and signed off on the response form. ;)

I wonder how many of those signatures were garnered by passing around the form while the "bubs" were doing hanger flying. I hope none, but I'm a realist. I've seen it done.
