Title: Volunteer Service Death Benefits

Yesterday at fire academy class, i was given a form to fill out that was a death benefits form. At the top, it listed that it applied to volunteer firefighters, EMS personnel and the Civil Air Patrol, yet Ive never seen this form in association with CAP and certainly have never been given one to fill out.

Anyone know anything about it.

What state are you in?  Also, what organization was the form itself from?

Mr MedTech,
The form you filled out refers to the following IL state law which cover death benefits "In the Line of Duty":

Line of Duty Compensation Act
(III.Comp. Stat. 2004, 820 ILCS 3151-315/5)
