Title: Obsolete Forms

Does anyone have the previous versions of CAP Index 0-9? I am interested in finding out when certain forms were rescinded and/or rendered obsolete. The last printed copy I have of the index is dated July 2004.

The bottom of each form has a date and when the old one should be stopped being used. That's some clue at least. You could also use  http://web.archive.org (http://web.archive.org) and snoop around the CAP sites as archived.

I'm tracking on the dating at the bottom of the forms, but that isn't helpful if the form has been rescinded.

How far back do you need to go? I have PDF indices back to about '03, and a couple of paper ones older than that.

Which form(s) are you looking for?

I have a bunch of forms that go back embarrassingly far as well. 
If you tells us which you want, we can probably recreate the whole trail back to the mid-90's or further.

Actually, I discovered that we had electronic versions back to '03 and it answered the mail. Someone was wanting a form signed that was rescinded 4 years and 4 days ago.

Thanks for the offers, though.

On a similar note, it would be handy, if only from a historian's perspective, to have a comprehensive list of obsolete forms with the date of their expiration.
