This is awesome. So speaking of NESA, my Squadron and Wing approved it. Now I have the medical and liability thing to send it, however, it is asking for a witness signature. I am an adult, who is suppose to be the witness? Or do I just send it in?

Walk over to a coworker or friend and say "Hey, watch me sign this, then sign here".
A witness signature is different than a parent's signature. It's to help prevent forged signatures and is one step below requiring a notary.
