What forms are needed to have a GAWG call sign if you already have a roa card?

I don't have the answer for GAWG, but I am sure your unit DC will have a good answer for you.
Usually you need a personal owned radio or get yourself attached to a unit radio in order to obtain a wing level callsign. In California for example there is a max number of members that can be attached to each unit radio, and therefore in some units they won't have allocations for new callsigns unless the member can come up with his/her own radio. Again your unit DC will be your best bet.

Does your unit have a Communications Officer? Have you asked him/her? Lacking that, have you asked your unit commander? Has the Q gone to Group?

Those people will likely have more, and better, information than most members here.

After reading 100-1 it appears that the issuance of a call sign is limited.

a.   Within CAP, each region, wing and national level station is assigned a unit tactical call sign. The tactical call sign maybe   
            suffixed with a serially assigned number. Serially assigned numbers will not exceed four digits.
b.   Numerically suffixed tactical call signs one through five will change with changes in staffing and are as follows:
       (1)     Commander
       (2)   Vice commander
       (3)   Chief of staff
       (4)   Director of Communications
       (5)    Chaplain


Now I have heard that (in my wing) if you have completed ACUT you can be assigned a call sign, however I have not been able to find any supporting information at the moment.  Talk to your units Comm officer, they should be more of a help.

The other question you need to ask yourself is why do you need a callsign? 
90% of the time we use functional designators instead (excluding aircraft, but not applicable in this instance), the other 10% your units callsign (or vehicle as appropriate) will suffice.

I have acut and bcut. I need a call sign for hoping net and practeing my comm skills.

Each wing has their own procedure. Ask your Comm officer what you need to do.

The same way you got your ACUT..  is probably how you get the call sign. Maybe even the same form with just more information. Like a serial # to a radio that will be used.

Callsigns are issued for "compliant" radio equipment either owned by CAP or the individual.
You just don't get issued a personal callsign without any radio equipment, IF you decide to buy some radio equipment make sure you talk with knowledge communications staff officers to ensure you buy something that will be allowed to be licensed.

In your wing. Each wing is different. In my unit we have at least four members with callsigns. There is only one VHF radio in the unit. Here in INWG if you complete ACUT, you get a callsign. No radio need be assigned.

Thanks.  I took ACUT a couple of weeks ago in Ft. Wayne, so I'm just waiting for the paperwork to go through.  With summer encampment last week, I figure that the wing people were probably kinda busy.  I'm not really in that big of a hurry.
