The CyBorg is destroyed

My PDO recently asked me for an updated CAPF 45, since apparently my old pink folder is falling apart, not surprising for multiple uses/moves over the past 19 years.

I checked at capmembers.org and it doesn't look like there's one "fillable-outable" online, at least not for Adobe.

Is there one that is "fillable-outable?"  I do not want to submit our long-standing, long-suffering PDO to my subpar bad terrible awful physician-like virtually unreadable handscratchings if I can avoid it.

CAP form 45 is available in word format that can be updated.  I have been asking national HQ for an auto fill form to be linked to national database which would allow for entering CAPID and all other info would be automatically be filled in. After it can be printed out and place in personnel file folder. Maybe someday.

Eclipse, that is the CAPF 46 not the 45.

While the OP request was for a non-Word version, there isn't an official PDF version here: http://www.capmembers.com/forms_publications__regulations/forms/

So, either Word or find someone with full blown Adobe to make it from the Word version.

Errant past, corrected above.

What's the issue with the Word version? It's fully interactive.

If you don't have Word, it can be opened with Open Office.

Before I called it quits I was the PDO/Admin/sqdn GoFer.  I downloaded the Form 45 in word format and filled it out for every member of the squadron and updated as necessary but at least monthly.  I had a folder for every member and items not reflected on on the Form 45 such as Pilot Certificates/Medicals/conference attendance, etc. I would either scan and keep in their folder.  It took a bit of work to set up but once up and running was easy to keep.  In the squadron folder I had a spreadsheet listing every member vertically and the requirements for each level horizontally and the date each achievement was met.  Allowed for squadron status at a glance.  Again, work to start but easy to keep (if kept up to date).  Another spreadsheet set up along the same lines tracked all the ES training/requirements/achievements of the members.

ARMY:  Because even the Marines need heros.
CAVALRY:  If it were easy it would be called infantry.

Thanks, all.  I recently rebuilt my computer and hadn't reinstalled my dusty old Office 97 when I tried to open a CAPF 45.

Now I hope my dusty old Office 97 can be coaxed into opening more modern forms. :)

You'll probably need this: http://www.microsoft.com/en-us/download/details.aspx?id=3

Office 97 is pretty creaky, you might be better off with Open Office for more modern formats and features.

Open Office needs the 45 Doc be unlocked before you can use the fill-able fields.  PM me if you need that.
