Yesterday at fire academy class, i was given a form to fill out that was a death benefits form. At the top, it listed that it applied to volunteer firefighters, EMS personnel and the Civil Air Patrol, yet Ive never seen this form in association with CAP and certainly have never been given one to fill out.

Anyone know anything about it.
"Corpsman Up!"

"...The distinct possibility of dying slow, cold and alone...but you also get the chance to save lives, and there is no greater calling in the world than that."

What state are you in?  Also, what organization was the form itself from?

Please post your copy so we all can see what it looks like.  (just black out personal info)   ;)

Mr MedTech,
The form you filled out refers to the following IL state law which cover death benefits "In the Line of Duty":

Line of Duty Compensation Act
(III.Comp. Stat. 2004, 820 ILCS 3151-315/5)

Here's the complete URL for a PDF copy of the Law:

This law does cover CAP Members - the benefits are outlined within.

Hope that helps.
