CAPWatch to Fusion Table help

Okay, here is my current project.  I would like to take the CAPWatch text download and figure out a way to upload the specific data points of Member Name, Address, and Specialty into a Google Fusion Table.  The end goal is to be able to select Mission Pilot on the table and get a map in Google Earth/Maps that shows locations of all MPs.

I used to be able to download the access version.  Not having a necessity to use it for awhile, I didn't keep up with the text file version.  Does anyone know how to take the text file download and create one table with the CAPID, Member Name, Specialty, and Address in it?

You'll need to create a view out of multiple tables.
