Does anyone have a uniform issue form they can share or know where to find one?

You can use the CAPF 37, which is available in ORMS, under Printable documents at eh bottom of the menu.
Also, in ORMS, there is "Other Property" with "Expendable property" as the link. This creates a record in ORMS of expendable property, which includes uniforms, attached to member's CAPID. Items entered fall off after a year, IIRC.
You can also create a local issue form.
