Hope I haven't "pegged my stupid question quota" here, but my question is:

Does CAP have a form to keep track of senior member's performance, like an annual review form?

Yes, its' CAPF 40, CAP Performance Feedback Form. It's not required, but it can serve as a nice self-evaluation to discuss with your supervisor, or as an evaluation by your supervisor.

It looks like the CAPF 40 is hooked to the OE track, and not necessarily in general use.

I've had two group commanders ask me to prepare a CAPF 40 on myself, so some commanders are using them.

OE is certainly not in general use.
