Once qualifications have been met per CAPR-35-6, what paperwork must be submitted, what is procedure, to request ratings such as MO or Master Observer?

The tasks and missions, including FAM/Prep, etc., are submitted / considered / approved by SETs via
Operations Qualifications / SQTRs in eservices.

Once completed, the Unit CC or DOS submits the rating up to Wing for final approval.

Once approved at the Wing, you are qualified to act unsupervised.

The requisite badge is approved separately though a different module in eServices.

Go to Ops Quals and check all the tasks you met to qualify for the rating. It is kind of automatic as you fill in all the blanks per task online. Then at the bottom of the computer screen, click the submit button. This can be done by you, by the person you worked to achieve the tasks, or by the officer in charge if you were participating in a school where the skills were taught.

If you are submitting the tasks yourself, once submitted will send the tasks to the person that supervised you on those tasks for verification. If he / she verifies, the system will submit the tasks to your squadron ES officer or Squadron CC for authorization. If they authorize, then the system will submit those tasks to your Group CC and ES Officer who then approves. It is then elevated to Wing ES.

Although this explanation may seem too complicated, the only things you have to ensure is to 1) make sure your SQTR is properly signed, 2) scan and upload the SQTR into your ES Quals record, and 3) you fill out every task blank. Although some ES officers here may tell you it is not necessary to submit your signed SQTR online, sometimes doing this greases the process. I know of a few SET that are still denying they taught a task to some.
