Title: Modified ICS Forms?
Post by: Holding Pattern on March 20, 2018, 04:18:51 PM
In your wing, do you use modified ICS forms? I think I've seen about 7 different variations of personnel check-in forms now (not just CAP modified) and no standardization between them.

Yes - by design there is no "standard" process or form.

ICS is intended to be modified to suit local needs.

From the ICS Forms Booklet FEMA 502-2

ICS Form Adaptation

When agencies and organizations require specialized forms or information for particular kinds of incidents, events, or disciplines, it may be beneficial to utilize the essential data elements from a particular ICS Form to create a more localized or field-specific form.  When this occurs, organizations are encouraged to use the relevant essential data elements and ICS Form number, but to clarify that the altered form is a specific organizational adaptation of the form.
