PT pre-printed sheets

At one time, you could print the PT sheets from eServices with the cadet names on it. It also had their HFZ requirements dependent on their sex and age. How do you do that? I can't figure it out.

eServices > Cadet Programs > Cadet Promotions > Reports > Cadet Physical Fitness Training Report
