Is it necessary to be filled out or is it only for cadets that require or want certain OTC medications? If it is necessary what should be listed as the medicines that can be given to me?
Most likely, if it's not filled out and on file, you will not be given any OTC meds, including aspirin.
As for what meds should be listed, include any OTC meds you have taken over the past year - just my opinion.
