Can anyone tell me where to find an "AF Hold Harmless Form" for AFJROTC O-Rides?

It's specified in the MOA for AFJROTC O-Rides but I can't figure out where to get a copy and no one seems to know, including the commander of the JROTC squadron. I've tried contacting another local JROTC squadron that did O-Rides recently but they won't return my call.

All of the other documents mentioned in the MOA are included in the MOA expect the Hold Harmless form.

I'm familiar with CAP cadet O-Rides and CAPF9,  but I'm new to AFJROTC O-Rides. The MOA seems to be referring to an AF specific form.

Thanks, but that link is to an AF Hold Harmless Form to use Non-CAP airplanes on AF assigned missions.

What do I use for AFJROTC cadets?
