Title: What form would this be...

  Ok.  I have one of my recruits that is having some serious health problems.  He is fine to do drill and such, but will need excuse from most PT.  What form would need to be filled out in order to excuse him PT.  I am assuming that his parents, his doctor, and his squadron commander would need to sign it, right?  Also, is this form online where it can be printed off, or is it in an area only commanders can get to.  Thanks.

Take a look in CAPP 52-18 (http://level2.cap.gov/documents/u_082503084254.pdf), which is the pamphlet governing the Cadet Physical Fitness Test, as directed by reference in CAPR 52-16 "Cadet Programs Management".

Specifically, read page 6 and see attachment 1.  Notice on page 6 the use of the word "suggested" not "required" when referring to use of attachment 1.

I usually get a note from the cadet's doctor and staple it to the form.  Put the whole thing in his record.

  Thanks everyone.  We have already filled it out and are getting it signed by his doctor right now.
