This is a great article debriefing the helicopter crash of the Alaska State Troopers "Helo-1".  There is also a great video imbedded in the article of an interview with a pilot who flew into IMC on a night mission.  The article is geared towards helicopters but it's relevant to any IMC discussion.  One of the most relevant is pilot attitude and despite their high time and experience, the interesting decisions the pilots made, such as caging their attitude indicators because they were not believing what it was telling them.
