This is a nice summary of a now 12 year old study that examined why the pinacle of our pilot population (ATP's flying transport category aircraft in all weather) still have accidents.  Cognitive and procedural errors made by these very capable, concientious, current pilots look similar to what we might likely find in GA cockpits (including CAP).   I particularly liked the suggested 'remedies'. 

The slides might be used 'as is' to launch a discussion among pilots & aircrew some evening where aviation safety is the topic of the moment.
