CAPC 215

Is an image of a CAPC 215 available online anywhere?  Is it still used in conjunction with the AEO Specialty Track?  I found a 1998 version of the specialty track guide that references the CAPC 215 specifically, but the latest version just refers to "the certificate for Technician, Senior, or Master Rating".

Yes.  The CAPC 215 is still used.  I don't have/can't find an image of it.  And I cannot expect it has changed over the past few years.  You get a copy when your Commander's Evaluation and Rating Certification Checklist is signed and submitted to CAP/ET.
