Title: Pressure to fly, mission incapable

Flight Safety Australia's discussion of pressures to fly (self induced, other induced) is pretty good.  Ditto for the systemic problems that setup the crew for failure.

It can happen just as easily for driving too.  This past spring, I had an event that I should have stayed put, but that "gotta get home" syndrome bit me hard.  I got lucky and went in the ditch to where I could get out.  I got lucky with only $35 worth of damage to my car.  If it would have been in some other parts of my drive home, it could have literally killed me.  That made for a wonderful Safety brief the next week. (I'm the safety officer, and I told on myself.)
