Where are the Form 5 and O-Ride Tests/Exams

I think this question is appropriate in Membership as it is part of my professional development, but not 100% sure.

I have searched and searched but cannot find the Form 5 Exam and the Orientation Ride Pilot Test. If anyone can post direct links or navigation paths, I would really appreciate it.

Do you happen to have the form numbers in "Forms" for the Form 5 Exam and the O-Ride Test?

I kind of expected to find tests and exams in Axis.

I'm pretty sure what the OP is looking for is the actual test, not the questionnaire, especially considering he's asking for tests.

The tests you need are located in AXIS. Go to Course catalog and search for CAPR 70-1 General Flight Exam, CAPR 70-1 Powered Flight Exam, and Cadet Orientation Pilot Endorsement Quiz Non ROTC. If you're also going for an ROTC orientation endorsement, then do the For ROTC quiz as well. Note, next year when you have to take these again, they will not be in the course catalog, instead you will find them in the Completed Courses section of the first page on AXIS.

This actually is NOT the current CAPF 70-5A and should not be used. When in doubt, always go to https://www.gocivilairpatrol.com/members/publications to get current up to date publications.
