Did CAPF 60-81 get forgotten about?

CAPR 60-1 refers to CAPF 60-81, but the form appears to be missing from the forms site. Any word on when this is getting corrected?

CAPR 1-2 indicates that "8.4. Forms. All forms, regardless of the issuing unit, must be prescribed in a directive publication. The prescribing publication states the purpose of the form and directs when to use the form, when to complete it, and how to submit it."

CAPF 31 no longer appears to be prescribed in a directive publication.

year 3 and maybe 1/3rd done.
