How does one do a safety report & review, CAPR 62-1 was no help

Mishap & Incident reporting is done in SIRS via eServices.

which was very recently updated.

If you are in a squadron, definitely talk with your squadron commander first.
If you are a squadron commander or higher, talk with your wing safety officer first.

Some wings have supplemental rules on mishap reporting. Pacific Region for example has a phone number to call before filing paperwork.
