High altitude affect of dental procedures -CASA Flight Safety article

Any dentists or medical folks on forum who can discuss the risks of dental denial during the "IMSAFE" self assessment and subsequent conversation with the FRO?   

FWIW, I've only turned up one (fatal) accident report in the NTSB DB that discusses dental issues.  Some of the referenced aviation dentistry articles say that the incidence of dental issues is on the upswing.  I've not seen any other discussion of this as a health issue, though it's easy to imagine.  I don't think I want to fly with a toothache.  Been there (on the ground), and didn't like it at all.  I think it would be really difficult to concentrate on flying with any of the symptoms discussed in the Flight Safety Australia article.
